Intervalul QT la pacientii cu accident vascular cerebral

Rezumat

Avand in vedere mortalitatea cardiaca crescuta la pacientii cu accident vascular cerebral (AVC), studiul actual isi propune o evaluare electrocardiografica atenta la acesti pacienti. Au fost investigati electrocardiografic 68 de pacienti cu AVC ischemic si hemoragic, observandu-se: alungirea semnificativa a intervalului QT (445±115 ms la pacientii cu AVC fata de 401±15 ms in lotul martor alcatuit din 20 de persoane sanatoase, p=0,047), cresterea dispersiei QT: QTd-diferenta dintre valoarea maxima si minima a intervalului QT (170±130 ms la pacientii cu AVC, fata de 38±11 ms in lotul martor, p=0,000312). Nu s-au obtinut deosebiri semnificative la pacientii cu si fara boala coronariana asociata pentru intervalul QT si QTd. La pacientii cu AVC se impune analiza atenta a intervalului QT, pentru a depista modificari predictive pentru aritmiile ventriculare si moartea subita, mai ales daca se asociaza si alti factori torsadogeni. Cresterea QTd in AVC nu se datoreaza patologiei coronariene concomitente.

Abstract:

Considering increased cardiac mortality in stroke patients, it is the aim of this study to evaluate ECG in those patients. 68 stroke patients underwent 12 lead ECG. QT interval prolongation (445±115 ms in stroke patients compared to 401±15 ms in the control group made of 20 healthy persons, whithout clinical cardiac or neurologic disease signs, p=0.047) and increased QT dispersion-the difference between the maximal and minimal QT interval duration (170±130 ms in stroke patients, compared to 38±11 ms in the control group, p=0.000312), were noticed. There were no significant differences between the QT interval duration and QTd in stroke patients with and without concomitent coronary disease. Stroke patients should undergo early carefully cardiac examination, to detect electrocardiographic changes like: QT interval prolongation, increased QT dispersion, which are predictive for ventricular arrhythmias and sudden death, especially if some other torsadogen factors interfere. QTd and QTdc increase, and consequentlly ventricular arrhythmia risk in stroke patients, is not caused by concomitent coronary disease.
